  • 3. A Water hazard Exclusions applies. Are there any water hazard on the property such as (ponds, lakes, streams, rivers) or other similar bodies of water?*

  • 15. Is there a leader, sweeper or check-in and check-out for each participant to confirm each rider has safely returned each day?*
  • 16. Are there any water hazards, cliffs, or drop off areas along the ride?*
  • 17. Are you required to carry Workers Compensation insurance to cover your employees, workers and volunteers?*

  • 20. Do you strictly adhere to the AMA Minor Waiver Requirements listed below?*

  • BOTH PARENTS MUST SIGN THE WAIVER (UNLESS ONE PARENT INDICATES THEY HAVE SOLE CUSTODY). IF NOT PRESENT AT THE EVENT A NOTERIZED WAIVER MUST BE PROVIDED AND SIGNED BY BOTH PARENTS. A GUARDIAN CANNOT SIGN FOR A MINOR PARTICIPANT.
